All attendees are requested to fill in and submit the below IX Pan-American Workshop Registration Form together with payment by registration dates.

If it becomes necessary to cancel your attendance to the conference, you are required to submit a written statement, name, address, phone number and email. Cancellations before June 1, 2018 will be charged a process fee of 25% of registration payment. Refunds will not be issued after June 2, 2018. Please make payment in US dollars only.
