VIII PanAm Registration

All attendees are requested to fill in and submit the below VIII Pan-American Workshop Registration Form together with payment by March 15, 2014 for Early Registration. Any registration received after March 15, 2014 will be subject to Regular Registration fee.

If it becomes necessary to cancel your attendance to the conference, you are required to submit a written statement, name, address, phone number and email. Cancellations before July 15, 2014 will be charged a process fee of 25% of registration payment. Refunds will not be issued after July 15, 2014. Please make payment in US dollars only.
